At Green Apple Home Health, we believe specialised exercise programs should be available to anyone. Our service has been carefully designed to provide the community with access to a personalised program at very reasonable rates.

At Green Apple Home Health, your needs are our priority. We offer a free initial consultation so you can get to know us and understand our approach, without any risk.  

If you decide to join us, billing is easy to set up and convenient to manage. Our members are not locked into a contract.

Medicare and insurance rebates

You may be eligible to receive a rebate on your Home Health fees.

Telehealth

If you are located in a telehealth eligible area, you may be able to claim your initial Home Health consultation through Medicare.

Chronic Disease Management (CDM) plans

If you have a chronic health condition*, your doctor can provide you with a Chronic Disease Management (CDM) plan to see an exercise physiologist. 

CDM plans are funded by Medicare and give you access to a maximum of five sessions each calendar year. You can use these sessions to work with an exercise physiologist through our Home Health program. 

Sessions run under CDM referrals are bulk billed, meaning you have no out-of-pocket expenses. 

*A chronic condition is one that has been (or is likely to be) present for six months on longer, for example, asthma, cancer, cardiovascular diseases, diabetes, musculoskeletal conditions and stroke.

NDIS

If you have an approved NDIS plan, you may be able to access funding for your exercise physiologist treatment costs. Your plan must specify services under one or more of the following categories:

We provide Home Health services to our NDIS clients at no out-of-pocket expense.

Department of Veteran Affairs

If you hold a white or gold veteran healthcare card, your Home Health fees will be bulk billed. You will need a valid referral from your doctor to get started.

Private health fund

Some private health funds have approved telehealth items for exercise physiology. If you are unsure, contact your fund directly and ask if they cover treatment via telehealth.
